School Vaccine Exemptions

New York allows one exemption from school immunization requirements — a medical exemption certified by a physician licensed in this state. This section has two template letters to send with your child’s DOH-5077, step-by-step instructions for filling them out, and a nine-page FAQ covering who decides, what a school may and may not ask you for, and what to do if you are denied.
